1. Entry is not insurance

A Malaysia Takaful or domestic medical card is not Russia travel medical insurance. Buy a policy that covers emergency treatment, transport and repatriation in Russia for the whole trip including arrival and departure. Match names to the passport, not the MyKad.
3. Paying with an international card
Malaysian Visa and Mastercard often pay hosted international merchants and still fail inside Russia. Pay before you fly. 3-D Secure from a Malaysian bank is normal. A declined 3DS is not proof that insurance is unavailable — check the bank SMS before retrying.
