Spanish citizens are not covered by EU health cards in Russia. Confirm e-visa or ordinary visa on the MFA portal and the Russian Embassy in Madrid.
1. Entry is not insurance
Tarjeta Sanitaria Europea does not work in Russia. Treat Russia as a separate insurance destination from Schengen.
2. What cover to buy
Buy travel medical insurance for the full stay including arrival and departure. If a consulate asks for EUR 30,000, provide the certificate the insurer issues — a booking confirmation is not that certificate.
3. Paying with an international card
Spanish Visa/Mastercard usually fail at Russian POS. Pay on hosted checkout. Bizum will not pay a Russian hospital.

- Charter or group invoices in Spanish must still match the passport Latin spelling.
- Already-in-Russia purchases can be declined or waiting-perioded.
